Medical Questionnaires

Sleep apnea and rhinosinusitis are two conditions that many Americans suffer from and often, may not even realize it. When left untreated, these conditions can lead to other serious physical and mental health issues. 

If you think you may be suffering from sleep apnea or rhinosinusitis, fill out one or more of the questionnaires below, and Dr. Matorin and his staff will go over the results with you at your appointment. From there, they will work with you to find a customized treatment plan suited for your needs.

Meet Dr. Matorin

Dr. Matorin is a highly trained otolaryngology and ENT specialist providing the latest medical and surgical treatment options for disorders of the ears, nose, and throat in both adults and children. He’s a strong advocate for patient education, founding our center around the idea of a holistic approach to medical care. He believes that patients should be highly involved in the decision-making process for their treatment, and Dr. Matorin spends a great deal of time with patients to make sure they fully understand their options.
